The patient interface is a web browser or an App. Through the interface, the patients answer a simple online digital questionnaire on a daily basis. The design of both the web browser and the App is user friendly and easy to use for all age groups.
Patients are guided through comprehensive set of questions relevant to the condition that is monitored. This enables the clinician to follow the progress of the patient’s condition – before, after, or between physician consultations. Furthermore, the history view allows the patient to self-monitor the progress of their own health.
All data is collected in real-time and is immediately visible to the healthcare professional. The patient interface can be setup to integrate with wearable and IOT devices via Bluetooth and submit data directly into the platform.
Through the Healthcare Professional (HCP) portal, healthcare professionals can monitor the condition progress of the relevant patient group.

Depending on the questionnaire answers, the patient responses will generate alerts: Red – clinical alert (Priority 1), yellow – clinical alert (Priority 2), or blue – reporting a non-response or partial response (Priority 3).
Based on the alert status, the HCP can reach out to the patient either via the chat function or potentially also via a video call if this feature is enabled.
The pathway defines the protocol which drives the questionnaire and thresholds used in the patient interfaces.
The protocol can easily be customized and designed to ensure relevant datapoints are collected. Furthermore the thresholds can be set by the clinician in accordance with the hospital guidelines for the specific therapeutical area.
